This guide page will show you how to get to the finale of Baldur’s Gate 3 through the Confront the Elder Brain quest. We describe where to meet the Netherian Brain and how to save Orefus.
Confront the Elder Brain is a game-closing quest – it leads straight to the end. So if you still have some side quests left, don’t start with them. Otherwise, the following pages of the guide will tell you how to complete the Confront the Elder Brain quest and complete Baldur’s Gate 3.

Initiation of the task
Return to Bhaal Temple and head in the opposite direction – a path you have not yet explored.
You’ll reach a boat – get on it and a cutscene will appear.
Next, walk along the path. There aren’t many side paths here, so just keep going. Finally you reach the ambush – defeat the enemies. They shouldn’t pose much of a threat to you even if you’re unprepared.
Keep moving until you meet Gortash. Talk to him.
Confronting the older brain
Follow him until a cutscene begins. The Nethermind will appear before you in all its glory and your task will be to take control of it. Passing the skill check reduces HP in the upcoming battle with the Netherbrain – otherwise it has no effect.
After the cutscene ends, talk to the emperor who saved you.
How to save Orpheus?
He will suggest you become an Illithid yourself to defeat the Brain. Alternatively, you can give him the stones so he can do it himself. Finally, you can free Orpheus, provided you have the Orphic Hammer – you could get it from Raphael if you made a pact with him or stole it from the House of Hope. In this walkthrough we will rescue Orpheus.
Choose the last dialogue option – the emperor will leave you alone and you can free Orpheus. Attack two red stones and then a cutscene will trigger.
You have to decide: either let Orpheus transform into Ilithid, thereby controlling the Nether Brain, or do it yourself. After that, go through the portal to Baldur’s Gate to complete the final task – Destroy the Elder’s Brain.
